29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged:  Warehouse dock #6 - On or about March 4, 2018, employee(s) were using a Heli forklift, model #CPC025-kull / serial #010250N1679) to lift and push on an approximately 800 lb. steel dock plate.  The load was not safely arranged in that the dock plate was being pulled off of the fork blades by a chain attached to a truck.,
