On or about March 09, 2018, at the residential construction site where employees were exposed to a struck-by hazard while operating a pneumatic nail gun without wearing eye protective equipment.

On or about March 09, 2018, on the residential construction site where employees were exposed to a fall hazard greater than 6 feet when performing roofing work on the roof of a two-story house without a fall protection system.
