29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about 03/12/2018 - CDF ROTO R005-A, the gap between the rotating table and a fixed motor assembly was not guarded.     (b) On or about 03/12/2018 - ROTO-7, the gap between the rotating table and the fixed operator's cabinet was not guarded.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    (a) On or about 3/12/2018 - OMGA Radial "Alabama" saw, flexible cords were connected in series to power equipment such as but not limited to the saw, floor fan, and dust vacuum system.       (b) On or about 03/12/2018 - Gatlin Gun, an extension cord with a 3-way splitter was connected to a 6-plug relocatable power tap (PRT) that was wired to a flexible cord that was hard-wired into a junction box.  Connected to the 3-way splitter were a cord for a portable vacuum system and another extension cord that was connected to the hot melt feeder controls.    (c) On or about 3/12/2018 - ROTO4/U-Cap Area, multiple flexible cords with RPTs were in use, such as, but not limited to, a 4-plug RPT was connected with a flexible cord to HB005-A Hot Box ROTO4; a 6-plug RPT was connected to two floor fans, tape machine, and another extension cord; and a 6-plug RPT that was connected to a radio, fluorescent light, an extension cord that was connected to the rotor injector machine, and another 6-plug RPT that was connected to a bag sealer, pager charger and another 6-plug RPT that was connected to two printers, a monitor, a router and a length gage machine.
