29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded: (a) New residential construction located on Lot #577, Front side of structure near street: On or about 03/08/2018, and at times prior, the employer exposed employees to amputation hazards while using an Hitachi Air Compressor, in that the guard that this equipment was manufactured with was not covering the belts and pulleys located in close proximity to the on/off controls.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: (a) New residential construction located on Lot #577, Second level: On or about 03/08/2018, and at times prior, the employer exposed employees to amputation hazards , in that the lower guard of the Makita circular saws was wedged to the upward position by a shim preventing the lower guard from recovering the blade after a cut was made while cutting joist and sheeting.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) New residential construction located on Lot #577, Second level: On or about 03/08/2018, and at times prior, the employer exposed employees to falls of 13-feet to 21-feet to the lower level, in that a fall protection system was not provided while employees were engaged in framing activities.
