On or about March 13, 2018, at the above referenced worksite, employees were performing residential roofing operations on sloped roof. Employees on the next lower level were exposed to falling/flying debris and not protected by hard hats. Employees were thereby exposed to head injuries. There is no abatement certification of documentation required for this item.
