29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems: a) 556 East Broadway: On March 13, 2018 and at times prior, employees were exposed to fall hazards of approximately 24 feet to the ground on a roof with a pitch of 5:12 while installing metal edging.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder: a) 556 East Broadway: On March 13, 2018 and at times prior, employees were exposed to fall hazards while using a ladder which did not extend 3 feet above the surface of the roof eave that was approximately 24 feet to the ground below. Employee was installing metal edging on a roof with a 5:12 pitch.
