29 CFR 1910.23(c)(9): (9) Portable ladders used on slippery surfaces are secured and stabilized; On or about March 5, 2018 at the job site located at 220 Country Club Ln, Warren, Ohio 44484; during the replacement of a flag pole bracket, the employer did not ensure that the portable ladder being utilized was properly secured and or stabilized while being place in a wet/icy mulch bed allowing it to kick out from underneath an employee while the employee was working at an elevated working position 10 feet above ground.
