29 CFR 1926.403(i)(2): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures:     a) West wall of building, on or about 2-27-18:  Employees were working in the immediate vicinity of an energized 12 capacity circuit breaker panel that was missing 5 circuit breakers, exposing the employees to energized parts inside the panel.     a) Northeast corner of building, on or about 2-27-18:  Employees were working in the immediate vicinity of an energized circuit breaker panel that was missing its cover, exposing the employees to energized parts inside the panel.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:    a) South wall of building, on or about 2-27-18:  Employees were operating various hand held power tools that were powered through receptacles that were not protected by a ground fault circuit interrupter (GFCI).     b) East wall of building, southeast corner, on or about 2-27-18:  Employees were operating various hand held power tools that were powered through receptacles that did not have a continuous path to ground due to improper internal wiring, rendering the GFCI protection inoperable.
