Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.   A) Dwaine and Cynthia Willett Children's Hospital, on or about March 9, 2018, employees were working on a flat section of a roof that had a parapet wall that was only 18 inches high without fall protection, exposing employees to a fall hazard of over 25 feet.
