29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Facility, 6001 Enterprise Drive, Export, PA:  On or about March 6, 2018 the point of operation on the Wegoma miter saw, SD17B, was not adequately guarded to prevent employees from contacting the blade during operation.  The guard on the saw did not completely close and was set that it left an unprotected area of 4 plus inches of the blade.  An employee was cutting vinyl and placed their hand within 1 inch of the blade.  The material and the employee's right hand was pulled into the saw.   The employee sustained an amputation of the right index finger and the repair of the middle, ring and little fingers as a result of contact with the saw blade.
