29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #1, Roof: On or about 03/14/2018, and at times prior, the employer exposed employees to fall hazards of 20-feet to the lower level, in that a fall protection system was not provided to employees engaged in roofing activities on a 12/12 pitched roof.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) New residential construction located on Lot #1, Front side at electrical service box: On or about 03/14/2018, and at times prior, the employer failed to inspect all electrical cords and equipment at the start of the workshift, in that extension/flexible cords powering tools on site had damaged strain relief which exposed the internal insulated wires under the protective casing of the cord.
