On or about April 12, 2018, in the following instances, exit doors were unable to be opened:  1.  Press Department:  The side exit door was dead-bolt locked during building occupancy.  2.  Behind the Roto Press:  The panic bar on the emergency exit door had been removed and the door would not open without a pin being removed.

29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical:  A.  On or about April 12, 2018, Facility-Wide:  The employer did not ensure that updated safety data sheets for each hazardous chemical used on site were available.  The company had a binder of outdated material safety data sheets for chemicals used on site, which included 2-butoxy ethanol; mineral oil, lithium soap and zinc; and dimethylethanolamine.  Exposure to these chemicals could cause eye, skin, respiratory tract irritation, skin burns and eye damage.
