29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    (a)  On or about March 1, 2018, and times prior to that date, employees were exposed to injuries from an unguarded bench grinder, with an abrasive wheel, at the point of operation where the employer failed to ensure an adjustable work rest was installed prior to use.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a)  On or about March 1, 2018, and times prior to that date, employees were exposed to injuries from an unguarded bench grinder with an abrasive wheel at the point of operation where the employer failed to ensure an adjustable tongue guard was installed prior to use. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a)  On or about March 1, 2018, and times prior, the employer had not developed and/or implemented a written hazard communication program to address the chemicals used in the workplace and the hazards associated with those chemicals.   A written hazard communication program shall include the following elements, as applicable:  1. An inventory of all hazardous chemicals used in the workplace; 2. Labeling of chemicals; 3. Maintaining of Safety Data Sheets for all chemicals, ensuring accessibility for employees; 4. Training; 5. Periodic review to address changes in the workplace.
