29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling:  a)  At the Facility, where electrical equipment not used as listed included:  1. Second Floor G3 Area, where a relocatable power tap was used to supply power to appliances including the Mr. Coffee 12 cup drip coffee maker.   2. Cubicle 8, where a relocatable power tap was used to supply power to appliances including the Hamilton Beach refrigerator and a small aquarium.   3. Room 261-C, where a relocatable power tap was used to supply power to appliances including the portable floor heater and a fan. (Item Corrected)  4. Room 285-A, where a relocatable power tap was used to supply power to appliances including the Sanyo refrigerator and the Oster microwave oven.  5. Room 283-B, where a relocatable power tap was used to supply power to appliances including the West Bend microwave oven and the Candle warmer.  6. Room 283-A, where a relocatable power tap was used to supply power to appliances including the Oster water kettle.  7. Room 242-E, where a relocatable power tap was daisy chained with an orange extension cord and used to supply power to appliances including the Simply Perfect microwave oven.

29 CFR 1910.157(e)(3):  The employer did not assure that portable fire extinguishers were subjected to an annual maintenance check:   a) Facility, where employees would evacuate the building in the event of a fire, annual inspections were not conducted for fire extinguishers including, but not limited to:  1. G-3 Area, where the Amerex fire extinguisher was equipped with an inspection tag dated January 2016.   2. Suite 260 hallway, where the Amerex fire extinguisher was equipped with an inspection tag dated January 2016.   3. G-8 Area, where the Amerex fire extinguisher was equipped with an inspection tag dated January 2016.   4. G-1 Cubicle Area, where the Amerex fire extinguisher was equipped with an inspection tag dated January 2016.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:   a) Room 283-B, where the power cord for the relocatable power tap was pulled from the strain relief exposing insulated conductors.
