29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: 70 Industrial Dr, Holden, MA On March 14, 2018 at the opening of the inspection, the employer did not have an energy control program to protect employees who performed tooling changes and servicing activities on numerous woodworking machines. There were no energy control procedures for large belt sander machines with pneumatic and electrical energy sources and employee training and periodic inspections were not provided to ensure employees performed proper hazard energy isolation and lockout.

29 CFR 1910.213(c)(3): Hand-fed ripsaw(s) did not have non kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator: 70 Industrial Dr, Holden, MA On March 14, 2018 and at times prior, employees operated table saws without non kickback fingers. The employer was not able to find any operational non kickback fingers in the shop to install on the saws.
