On or about March 14, 2018 two employees were working on a 5:12 pitch, two story residential roof at without any means of personal fall protection, guardrails, or safety nets, and were exposed to falls of approximately 20 feet to the ground below. b. On or about March 14, 2018 three employees were working on a 5:12 pitch, one story residential roof without any means of personal fall protection, guardrails, or safety nets, and were exposed to falls of approximately 9 feet to the ground below.

On or about March 14, 2018 the employer did not prepare, maintain or provide a written fall protection plan to employees engaged in residential new roof installations, to demonstrate that it is infeasible or it creates a greater hazard to use conventional fall protection equipment.
