29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  Parking Lot next to the building under construction:  On or about March 7, 2018, an employee was exposed to an electrical hazard while using an extension cord to power a circular saw while the outer jacket of the extension cord was pulled away from the plug exposing its internal wiring.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under1626.502(e).  Parking Lot next to the building under construction:  On or about March 7, 2018, two employees were exposed to a fall hazard, while installing soffit to the exterior of the building from a JLG 460SL aerial lift without wearing a Personal Fall Arrest System (PFAS).
