Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about March 15, 2018, at the construction site located at 4622 Coach Road, Columbus, Ohio, employees were performing roofing work on the top flat portion of the roof of a two story residential house. The flat roof portion was surrounded by sloped roof with a pitch of approximately 10 in 12 and approximately 20 feet in height. Employees were working without fall protection, thereby exposed to a fall hazard.
