29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     (a)  Canine Center of Excellence LLC, dba Rainbow Bridge Pet Crematorium, at 27292 Weld County Road 13, Johnstown, CO: On and preceding 3/14/18, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from acids or caustic liquids.  Employees used and dispensed Total Solutions Vanquish (containing quaternary ammonium compounds), Health Technology Kennel Kare (containing tetrapotassium pyrophosphate) and Clorox Bleach (containing sodium hypochlorite) to clean and disinfect work surfaces.  The liquids had a pH greater than 12.  The employer did not ensure that appropriate eye protection was used to prevent exposure to a splash of the caustic liquids.  This condition exposed employees to a corrosive liquid hazard.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a)  Canine Center of Excellence LLC, dba Rainbow Bridge Pet Crematorium, at 27292 Weld County Road 13, Johnstown, CO: On and preceding 3/14/18, where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Employees used and dispensed Total Solutions Vanquish (containing quaternary ammonium compounds), Health Technology Kennel Kare (containing tetrapotassium pyrophosphate) and Clorox Bleach (containing sodium hypochlorite) to clean and disinfect work surfaces.  The corrosive liquids had a pH greater than 12.  An emergency eyewash, emergency shower, or other means for quick flushing of the eyes was not provided in the work area.  This condition exposed employees to a corrosive liquid hazard.

The employer operated a B&L model BLP1000MS Crematory Incinerator.  The incinerator was fueled by natural gas and was powered by electricity.  The incinerator had multiple energy sources, including thermal and electrical energies.  The employer hired an outside contractor to perform maintenance upon the incinerator.  The employer did not inform the outside contractor of their lockout/tagout procedure.  This condition exposed an employee and an outside contractor to uncontrolled energy hazards.

Employees performed tasks at the crematorium, including handling bones and ash and processing bones.
