On or about March 15, 2018, located at 2623 State Route 605 in Galena, Ohio, the employer did not initiate and maintain programs for the safety and health of employees performing roofing and siding work while exposed to falls from a barn roof, ladders, chicken ladders, etc. up to approximately 40 feet.

On or about March 15, 2018, located at 2623 State Route 605 in Galena, Ohio, employees climbed a chicken ladder on the roof of a barn that did not extend from the roof peak to the eave, thereby exposing the employees to a fall hazard of approximately 22 to 34 feet.

Crossbraces shall not be used as a means of access.    a. On or about March 15, 2018, located at 2623 State Route 605 in Galena, Ohio, employees climbed over the top rail of a personnel basket that was attached to the forks of a JCB rough terrain forklift in order to gain access to the inside of a barn, thereby exposing the employees to a fall hazard of approximately 35 feet.
