Employees were exposed to struck-by hazard.  The employer did not provide refresher training to the designated powered industrial truck operator who was involved in a unsafe incident on March 8, 2018, before allowed to get back on and operate the powered industrial trucks (Sard mobile crane and Sit-down forklifts), on or about March 15, 2018.
