29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a. On or about March 14, 2018, at the above addressed jobsite, the company installed an air duct riser through floor openings that measured five (5) inches in depth by twenty-four (24) inches in width and did not install a cover over the open sides, thereby exposing employees to injuries associated with slip, trip, falls, and struck-by hazards.
