29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. The employer is failing to ensure that all employees operating powered industrial trucks at the facility have received adequate training as outlined in this paragraph of the standard. Some employees have not been provided the "hands-on" (practical) portion of the training prior to being allowed to operate powered industrial trucks in the facility. This was most recently documented at 340 Victory Lane, Lincoln, Nebraska on or about March 5th, 2018 when an employee was operating a "Walkie" forklift and turned a corner and collided with another forklift. The operator of the "walkie" forklift sustained a broken leg.
