29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: On or about March 20, 2018, at 2525 Davie Road Suite 330 Davie, FL 33317, an employee performing manufacturing of liquid caplets using an automatic machine was exposed to noise dose of 102.6% or an equivalent of 90.1 dBA which was capable of causing permanent hearing loss. Monitoring was performed for 480 minutes of sampling time.
