29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a)  Press Shop; On or about March 13, 2018 and at times prior thereto, where documented procedures were not utilized for the control of potential hazardous energy, employees were exposed to potential struck-by, and amputation hazards, in that; the employees were allowed to enter the 800 Ton Fine Blanking Press(s) fenced off danger area to perform housekeeping without locking out the machine with the Lockout procedure posted on the gate.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically contain specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:   a)  Production Floor; On or about March 13, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies, in that, the employer failed to ensure that the machine specific lock out procedures including but not limited to the #2 Laser Welder had sufficient detail and adequate guidance that authorized employee(s) could clearly understand how to safely and effectively utilize hazardous energy control measures for the machine while performing servicing and/or maintenance activities. Employees were required to work on the equipment with procedures that were generic, vague, and did not list 1) types and locations of machine or equipment operating controls; 2) types and locations of energy isolation devices; 3) and sequence or steps to perform the lockout.  b)  Production Floor; On or about March 13, 2018 and at times prior thereto the employer exposed employees to potential hazardous energies, in that, the employer failed to ensure that the machine specific lock out procedures including but not limited to the 800 ton Fine Blanking Presses had sufficient detail and adequate guidance that authorized employee(s) could clearly understand how to safely and effectively utilize hazardous energy control measures for the machines while performing servicing and/or maintenance activities. 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a)  Production Floor; On or about February 1, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that the employer failed to conduct the required periodic inspection of the energy control procedures for machinery with multiple energy sources which includes but is not limited to the #2 Laser Welder, 800 Ton Fine Blanking Presses, and the 1600 Ton Press. Employees were required to work on these machines with procedures that had not been audited periodically as required by the standard so that any deviations or inadequacies would be discovered and corrected.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:   a)  Press Shop; On or about March 13, 2018 and at times prior thereto, the employer exposed employees to potential hazardous energies in that employees were allowed to enter the 800 Ton Fine Blanking Press(s) fenced off danger area to perform housekeeping without all employees applying a personal lockout device on the energy control lockout point(s).

29 CFR 1904.41(a)(1): Annual electronic submission of part 1904 records by establishments with 250 or more employees. The establishment had 250 or more employees at some time during the previous calendar year, and this part required the establishment to keep records, but the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  a) DAS North America, Inc. located at 840 Industrial Park Blvd., Montgomery AL: On or about March 13, 2018, and times prior thereto, the employer failed during calendar year 2016, to electronically submit information from the OSHA Form 300A or equivalent by 12/15/2017. The establishment employed approximately 450 employees and was classified in the North American Industrial Classification System as 336360 during calendar year 2016.
