29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck is dismounted and within 25 ft. of the truck still in his view, the load engaging means was not fully lowered, controls neutralized, and the brakes set to prevent movement. On or about 02/21/2018, employee operating Raymond Stand-up Forklift, Model #425-C35TT, Serial #425-16-4899, did not ensure that controls were neutralized, and right foot was removed from pedal to prevent movement prior to dismounting.

29 CFR 1910.178(n)(1): All traffic regulations were not observed, including authorized plant speed limits. The truck was not kept under control at all times. On or about 02/21/2018, Employee operating Raymond Stand-Up Forklift, Model #425-C35TT, Serial#425-16-48992 to return an empty pallet to the warehouse area. Traffic regulations were not observed in that the truck was not kept under control at all times exposing employee to fall and struck-by hazards.
