29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a) U.S. Postal Service, Northern PDCF, 8409 Lee Hwy, Merrifield, VA 22081: On or about March 20, 2018, and times prior thereto, each work rest on the repair shop DeWalt abrasive wheel bench grinder used to debur metal was not adjusted, leaving a gap up to approximately 5/16" from the wheel, exposing employees to struck-by and caught-in hazards.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate powered industrial trucks:  a) U.S. Postal Service, Northern PDCF, 8409 Lee Hwy, Merrifield, VA 22081: On or about March 20, 2018, and times prior thereto, each employee that operated powered industrial trucks was not trained and evaluated, exposing employees to struck-by and other hazards.  No abatement certification or documentation required. CORRECTED DURING INSPECTION.  United States Postal Service was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1910.178(l)(1)(i) which was contained in OSHA inspection number 1143738, citation number 1, item number 1, issued on May 20, 2016 with respect to a workplace located at One Floretta Place, Raleigh, NC 27676 and was affirmed as final order on June 9, 2016.

29 CFR 1904.41(a)(1): Annual electronic submission of part 1904 records by establishments with 250 or more employees. The establishment had 250 or more employees at some time during the previous calendar year, and this part required the establishment to keep records, but the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  a) U.S. Postal Service, Northern PDCF, 8409 Lee Hwy, Merrifield, VA 22081: On or about March 20, 2018, and times prior thereto, the employer failed during calendar year 2017 to electronically submit information from the OSHA Form 300A or equivalent by December 15, 2017. The establishment employed approximately 830 employees and was classified in 491110 NAICS during calendar year 2016.  No abatement certification or documentation required.
