29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At residential construction worksite located at 120 Briar Valley, Blue Valley, Missouri. Employees engaged in roofing activities were exposed to a fall hazard of 20-25 feet from the upper level to the surface below. A complete guardrail system or other means of fall protection was not provided.

29 CFR 1926.501(b)(14):  Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  At the residential construction site located at  120 Briar Valley, Blue Springs, Missouri. Employees were exposed to a fall hazard while working on an unguarded stairwell, approximately 10-12 feet above the surface below, without the use of guardrail systems, safety net systems, or personal fall arrest systems.
