The employer was using an all-terrain vehicle with a wood pallet as a supported scaffold not designed or approved by the manufacturer.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. At the residential construction site located at NE Alhambra, Lee's Summit MO 64064: An employee was exposed to a fall hazard. The employer was operating an all-terrain vehicle with a wood pallet as a supported scaffold; and on top of the pallet was a type A ladder. An employee was working on the ladder exposed to a fall hazard.
