29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) On or about March 13, 2018 at the residential construction site located at 2208 SW Park Ave, Blue Springs, MO 64015 Employees were exposed to fall hazards. Employee were performing reroofing operations at an elevated height of 9.2 feet without any means of fall protection. Employees were observed working along the sides and back of the site without any personal fall protection or fall protection systems. b) On or about March14, 2018 at the residential construction site located at 2208 SW Park Ave, Blue Springs, MO 64015 Employees were exposed to fall hazards. Employee were performing reroofing operations at an elevated height of 9.2 feet without any means of fall protection. Employees were observed working along the sides and front of the site without any personal fall protection or fall protection systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: At a residential construction site located at 2208 SW Park Ave, Blue Springs, MO: Employees were exposed to fall hazards. The employer failed to protected employees from fall related hazards in that employees were not trained on recognizing fall hazards and avoidance of the unsafe conditions. This was demonstrated when employees were observed working at a height 9.2 feet with the use of wood 2x4s to provided a step to place their feet as not to slide.

The employer failed to provided safe access to an upper landing for employees. Employees were observed using a portable ladder to gain access to a roof, and that ladder was only placed 2 feet above the upper landing that was being accessed.
