29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  At the residential construction site located at 605 S Broadway, Oak Grove, Missouri:  Employees were exposed to struck-by hazards. Employees were engaged in roofing activities with the use of nail guns where eye protection was not provided while installing roofing materials.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  At the jobsite located at 605 S Broadway, Oak Grove, MO:  Employees engaged in residential framing activities were exposed to an electrical shock hazard.  The employer failed to protect employees from electrical shock hazards by allowing the use of flexible cords and cables that were visibly damaged. An extension cord being used to provide 120 V power to a SKILSAW brand circular saw had exposed internal wiring.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  At a construction site located at 605 S Broadway, Oak Grove, MO: Employees were exposed to electrical shock hazards. The employer failed to protect employee from electrical shock in that equipment was being used without a ground pin. A SKILSAW brand circular saw was connected to an extension cord and did not have a ground pin to ensure the path to ground was permanent, continuous and effective.

Employee were performing work on a steep roof and were not protected from falling by the use of guardrails, safety nets, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a worksite located at 605 S Broadway, Oak Grove, MO: Employees were exposed to fall hazards. The employer failed to provided safe access to an upper landing for employees. Employees were observed using a portable ladders to gain access to a roof, and that ladder was only placed 2 feet above the upper landing that was being accessed.
