29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which when subjected to heat, might produce flammable or toxic vapors: (a) Dragon Vacuum Tanker - Model 150 BBL: On or about March 14, 2018, employees conducted hot work maintenance on the rear exterior of a 6,300 gallon vacuum tanker trailer, to remove a stuck and leaking drain plug from its threaded fitting. This hot work was performed without insuring the tank had been thoroughly cleaned of flammable materials or other substances that might produce flammable or toxic vapors when exposed to heat. This exposed employees to a fire hazard and struck by hazards from an explosive reaction of heated substance(s) within the vacuum tanker trailer.

29 CFR 1910.252(c)(11)(i): Manufacturer's Instructions. In the use of cleaning materials, because of their possible toxicity or flammability, appropriate precautions such as manufacturers instructions shall be, but were not followed. (a) Dragon Vacuum Tanker - Model 150 BBL: On or about March 14, 2018, employees conducted hot work maintenance on the rear exterior of a 6,300 gallon vacuum tanker trailer, to remove a stuck and leaking drain plug from its threaded fitting. The employer did not consult with manufactures instructions for the cleaning chemicals (soapy waste water) in the tank. This hot work was performed without insuring the tank had been thoroughly cleaned of flammable materials or other substances that might produce flammable or toxic vapors when exposed to heat. This exposed employees to a fire hazard and struck by hazards from an explosive reaction of heated substance(s) within the vacuum tanker trailer.

29 CFR 1910.252(a)(3)(ii): All hollow spaces, cavities, or containers were not vented to permit the escape of air or gases before preheating, cutting, or welding was performed: (a) Dragon Vacuum Tanker - Model 150 BBL: On or about March 14, 2018, employees conducting hot work maintenance on the rear exterior of a 6,300 gallon vacuum tanker trailer, to remove a stuck and leaking drain plug from its threaded fitting, did not properly vent the vacuum tanker to allow for the escape of air and potential flammable vapors prior to starting hot work. This exposed employees to a fire hazard and struck by hazards from an explosive reaction of heated substance(s)/vapor within the vacuum tanker trailer.
