29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  a) On or about March 15, 2018 and at times prior thereto employees were working from an aerial lift 13 feet high, with no lanyard connecting their harness to the aerial lift basket. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about March 15, 2018 and at times prior thereto employees were installing windows on the second floor exposed to fall hazards of 13 feet from an open and unguarded window opening.  b) On or about March 15, 2018 and at times prior thereto an employee, who was installing windows, exited his aerial lift onto the roof 13 feet high with no fall protection.  NOTE-Abatement certification is required for this item.
