29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Boot Assembly machines C01, C02, C03, and C04; On or about February 16, 2018 and at times prior thereto, employees were exposed to caught-in and crushed-by hazards while operating boot assembly machines C01, C02, C03, and C04, in that the employer failed to ensure that guarding was present to prevent employees from placing their hands inside the danger zone during the operating cycle.
