29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  On March 15, 2018, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 22 feet above the ground.    Paul Kelsey dba Kelsey Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 900808, citation number 2, item number 1 and was affirmed as a final order on June 11,2013, with respect to a workplace located at 1030 N. Elmwood Ave., Oak Park, IL 60302.    Paul Kelsey dba Kelsey Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 282274, citation number 1, item number 1 and was affirmed as a final order on April 6, 2012, with respect to a workplace located at 1432 Park Ave., River Forest, IL 60305.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:      a)  On March 15, 2018, employer did not provide training to each employee in order to recognize hazards associated with working at an elevation and to understand procedures to control or minimize those hazards.    Paul Kelsey dba Kelsey Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 900808, citation number 1, item number 1 and was affirmed as a final order on June 11, 2013, with respect to a workplace located at 1030 N. Elmwood Ave., Oak Park, IL 60302.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
