CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system: (a) Complete Contracting LLC, at 13801 Grant Street, Thornton, CO: On or about 3/2/18, the employer did not ensure that anchorages used for attachment of personal fall arrest equipment were designed, installed, and used as part of a complete personal fall arrest system. Complete Contracting LLC used a saw on 3/2/18 to cut approximate three foot by four foot holes in steel decking for roof access hatches of three buildings. An employee wrapped a 3/8" Grade 43 steel chain around a steel beam and connected the chain to itself using a non-locking carabiner. A self-retracting lifeline was connected to the chain using a locking carabiner. The chain and non-locking carabiner were not designed as part of a complete personal fall arrest system. This condition exposed employees to an approximate fifteen foot nine inch fall hazard.

29 CFR 1926.502(i)(4): Employer did not ensure that all covers were color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: (a) Complete Contracting LLC, at 13801 Grant Street, Thornton, CO: On and preceding 3/3/18, the employer did not ensure that all covers were color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard. Complete Contracting LLC used a saw on 3/2/18 to cut approximate three foot by four foot holes in steel decking for roof access hatches of three buildings. The employer did not color code or mark the plywood used to cover the holes. This condition exposed employees of Complete Contracting LLC, employees of The Whiting-Turner Contracting Company, and employees of Keenan, Hopkins, Schmidt and Stowell Contractors Inc to an approximate fifteen foot nine inch fall hazard.
