29 CFR 1910.1001(j)(3)(i): Building and facility owner(s) did not determine the presence, location, and quantity of asbestos-containing material (ACM) and/or presumed asbestos-containing material (PACM) at the work site. Employer(s) and building and facility owner(s) did not exercise due diligence in complying with these requirements to inform employer(s) and employee(s) about the presence and location of ACM and PACM.     On or about March 14, 2018, employee(s) working in the basement were exposed to broken and friable asbestos-containing floor tiles at the following locations in the basement:     a.  Beige floor tile located on the walkway by the cleaning supplies, contained 15% Chrysotile Asbestos.  Black mastic on the tile contained 8% Chrysotile Asbestos;   b.  Beige floor tile located by the staircase and the worktable contained 16% Chrysotile Asbestos.  Black mastic on the tile contained 0.2% Chrysotile Asbestos;   c.  Green floor tile located along the south wall contained 10% Chrysotile Asbestos.  Black mastic on the tile contained 0.2% Asbestos;   d.  Green floor tile located along the south wall contained 12% Chrysotile Asbestos.  Black mastic on the tile contained 2% Chrysotile Asbestos.     Employees were not informed about the presence and the location of the asbestos-containing material.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
