29 CFR 1910.305(g)(1)(iv)(A):   Flexible cords and cables were used for purposes prohibited by subparagraphs (a) through (f) of this paragraph in the following location, flexible cord was used in lieu of permanent wiring:      Batch Plant, throughout:  Orange extension cords were piggy-backed throughout the plant to energize fixed equipment such as a diesel pump, heaters, and lighting.  Fire hazard.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors:   Batch Plant, Trailer:  A series of orange extension cords were run through a hole in the external wall of a trailer to energize a diesel pump, lighting and a heater throughout the plant.  Fire hazard.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:                       A written hazard communication program was not developed for employees that used hazardous chemicals such as Portland cement and various admixtures to make concrete.  Employees were exposed to chemical hazards.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:                 Batch Plant:  Employees were not trained on the new label elements (e.g., pictograms and signal words) and the new SDS format, as required by the revised Hazard Communication Standard.  Employees used hazardous chemicals such concrete admixtures, Silica, and Portland cement.  Employees were exposed to chemical hazards.
