29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  At 1362 State Highway H, Sikeston, MO, specifically inside of the Steel Cutting Area, on 03/20/18, the danger area belonging to the following machines was not guarded:  a/  Piranha Ironworker, m/n P50, specifically the two stock holders on either side of the angle shear.  b/  Niagara Shear, m/n IL-8, specifically the continuous stock holder running the width of this machine.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation (P.O.O.) of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  At 1362 State Highway H, Sikeston, MO, specifically inside of the Steel Cutting Area, the P.O.O. belonging to the Niagara Shear, m/n IL-8, was inadequately guarded.  The height of the fixed, barrier guard above the stock table was approximately 1.5 inches.  The blade was located approximately three inches behind the guard.  This violation was observed on 03/20/2018.

29 CFR 1910.252(b)(3): Employees exposed to the hazards created by welding, cutting, or brazing operations were not protected by personal protective equipment (PPE) in accordance with the requirements of 29 CFR 1910.132:  At 1362 State Highway H, Sikeston, MO, specifically inside of the Welding Area, welders were not wearing adequate PPE while performing hot work.  Two welders were not wearing a FR rated jacket or pants.  Another welder was wearing a FR rated jacket but no pants while welding the underneath side of a trailer.  This violation was observed on 03/20/2018.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four (4) business hours:  At 1362 State Highway H, Sikeston, MO, the employer was unable to produce the OSHA 300 Log and OSHA 300A Summary Sheet for calendar years 2014, 2015, 2016, 2017, and year-to-date 2018.  The request was made at approximately 8:30 a.m., 03/20/2018.  The OSHA 300 Log for calendar year 2015 was the only document produced by the end of the inspection four hours later.  This violation was observed on 03/20/2018.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  At 1362 State Highway H, Sikeston, MO, the employer did not develop a written hazard communication program that included a list of every hazardous material used by the exposed employees, container labeling (f), safety data sheet management (g), and how employee training was to be accomplished.  Employees utilize hazardous chemicals such as, but not limited, to Aeromatic 100/150 Thinner, Lacquer Thinner #10, Sher-Kem Paints, and Xylene Thinner products in the designated spray booth.  This violation was observed on 03/20/2018.
