Annual review of the program and documentation (see 1910.1030(c)(1)(iv)), b. Exposure determination (note: if all support staff are considered to have potential occupational exposure, then the plan should state that), (see 1910.1030(c)(2)(i)), c. Handling of regulated waste if generated (see 1910.1030(d)(4)(iii)), d. Hepatitis B vaccination and declination (see 1910.1030(f)(2)), e. Post exposure evaluation and medical procedures (see 1910.1030(f)), f. Training program, including annual re-training, and documentation of all training (see 1910.1030(g)). Abatement documentation is required for this violation.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available within 10 working days of initial assignment to all employee(s) with occupational exposure (Note: Does not apply to employees who have previously received the complete hepatitis B vaccination series, for whom antibody testing has indicated immunity, or for whom the vaccine is contraindicated for medical reasons): Hepatitis B vaccination was not offered to the support staff who provide services to customers in their homes (Note: the vaccination is not required to be accepted, but a declination form must be signed upon refusal. Training is required to be provided to employees with potential exposure, including the risk of potential infection.) Abatement documentation is required for this violation.
