29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:    a) Job Site - A ladder was not provided for access to a tubular welded frame scaffold, employees climbed the horizontal members of walk-thru scaffold frames to access the platform, on or about March 15, 2018.    No abatement certification or documentation required.

29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph:    a) North Side of Scaffold - An employee was exposed to an approximate 20 foot fall hazard to the ground below while working from a scaffold where the top edge height of the toprail was installed less than 38 inches above the platform, on or about March 15, 2018.     b) South Side of Scaffold - An employee was exposed to an approximate 20 foot fall hazard to the ground below while working from a scaffold that was missing a mid-rail and the top edge height of the toprail was installed less than 38 inches above the platform, on or about March 15, 2018.    No abatement certification or documentation required.
