29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about 3/19/2018, at 3435 Oakwood Road, Oakwood, GA: Where an employee operated a powered industrial truck with safety defects and deficiencies such as but not limited to damage and broken propane tank securing brackets, damage chain pulley, broken air filter cover, damage backrest cage, and handbrake on the lift did not hold the equipment in place on level ground. The employer failed to remove the powered industrial truck from service until the repairs could be made.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not adequately provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 3/19/2018, at the carpet separator equipment in the production at 3435 Oakwood Road, Oakwood, GA: Employees operating and working around the rotary lock area of the carpet separator were not provided adequate guarding from the rotating blades of the rotary lock section of the equipment. The employer failed to ensure the guarded material stayed secured in place to protect the employees from rotating parts.
