29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On March 9, 2018, there were no frequent and regular inspections of the job site, materials and equipment being performed by a competent person in that scaffolding and/or scaffolding structures were not being maintained and utilized in a safe condition.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, shall not be used on top of scaffold platforms to increase the working level height of employees. On March 9, 2018, located on the northeast side of a commercial structure, an employee was observed performing work while standing on top of a pile of insulation panels on a scaffold working level platform. Potential fall heights were measured to be greater than 46 feet above the ground below.

29 CFR 1926.503(b)(1): The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training. The employer did not have written certification of fall protection training for employees performing work from a tubular welded frame scaffold that were exposed to fall hazards at a height greater than 46 feet above the ground below.
