29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a. On or about March 20, 2018, at the above referenced worksite, employees were performing residential painting operations from ladder in excess of 20 feet in height, from ladders placed on sloped roofs, and from ground level. Employees working from each next lower level were not protected from head injuries by protective helmets. Employees were thereby exposed to head injuries from falling/flying tools/materials. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about March 20, 2018, at the above referenced worksite, employees were performing residential painting operations from a sloped roof. Employees were not utilizing fall protection while working on the roof. Employees were thereby exposed to falls of approximately 10 feet. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees were gaining access/egress to the working surface from an A-frame ladder that was not properly deployed. The employees leaned the A-frame ladder against the wall, and mounted the ladder. Employees were thereby exposed to falls of approximately 20 feet. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
