OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard:  a. On or about 03/30/2018 and times prior, employees were working on the ground in an area where heavy equipment such as, but not limited to, dump trucks and loaders, were operating exposing employees to a struck-by hazard.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a. The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections for the maintenance of the McCloskey Trommel model 621RE including the clearing of jams of construction and demolition landfill process debris.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Assigned storage spaces were not located where cylinders would not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  a) Coastal Landfill maintenance area - the oxygen and acetylene compressed gas cylinders were stored within 20 feet of a 55-gallon drum of used oil and two 5-gallon containers of gasoline.  b) Coastal Landfill maintenance area - Oxygen and acetylene compressed gas cylinders were not secured from falling.    c) Coastal Landfill maintenance area -  Oxygen and acetylene compressed gas cylinders were stored together.
