29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a) On or about March15, 2018, for the employee who sustained amputation injuries while aligning belting material in the left stretcher of clam shell press number two where the employees left fingers were caught-in the left stretcher arm clamp in the press work cell located at 1998 North Ninth Street, Wahpeton, North Dakota.       (b) On or about March 21, 2018 and at times prior to that date, for the employees exposed to caught-in and/or crushing hazards where the upper left and right stretcher frame of clam shell press number two were operated in auto dual tilt down (closed) in the press work cell located at 1998 North Ninth Street, Wahpeton, North Dakota.      (c) On or about March 21, 2018 and at times prior to that date, for the employees exposed to caught-in and/or crushing hazards where clam shell press number two was operated in auto down (closed) in the press work cell located at 1998 North Ninth Street, Wahpeton, North Dakota.      (d) On or about March 21, 2018 and at times prior to that date, for the employees exposed to caught-in and/or crushing hazards where the left and right stretcher arms of clam shell press number two were operated in auto down (closed) in the press work cell located at 1998 North Ninth Street, Wahpeton, North Dakota.
