29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents: (a) Martin Olmedo, dba Olmedo Roofing, at 901 through 906 Lucca Drive, Evans, CO 80620: On and before February 27, 2018, the exposing employer failed to ensure that eye and face protective equipment were used when machines or operations presented potential eye or face injury from physical agents. Employees were using hand tools and pneumatic nail guns to perform roofing work without safety glasses. This condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems: (a) Martin Olmedo, dba Olmedo Roofing, at 901 through 906 Lucca Drive, Evans, CO 80620: On and before February 27, 2018, the exposing employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems. Employees were roofing a two story apartment buildings without the use of fall protection. This condition exposed employees to fall hazards of approximately 20 feet while working off a roof with a 6:12 pitch.
