Employees were using hand tools and pneumatic nail guns to perform roofing work without safety glasses.  This condition exposed employees to the hazard of projectiles to the eyes.

Employees were roofing a two story apartment buildings without the use of fall protection.  This condition exposed employees to fall hazards of approximately 20 feet while working off a roof with a 6:12 pitch.
