On or about March 21, 2018, at the above referenced worksite, employees were performing residential roofing operations on sloped roof. Employees on the next lower level were exposed to falling/flying debris and not protected by hard hats. Employees were thereby exposed to head injuries. There is no abatement certification of documentation required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. a. On or about March 21, 2018, at the above referenced worksite, employees were performing residential roofing operations on sloped roof. Employees were provided a ladder for gaining access/egress to the next higher working surface. The ladder was not readily used, but posed a hazard if inadvertently used by an employee, because the ladder was not extended at least 3 feet above the surface being mounted. Employees were thereby exposed to fall of approximately 8 feet. There is no abatement certification of documentation required for this item.
