Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a)  8th & Samson, Philadelphia - On or about 2/26/18, employees performing roofing work were not provided with an adequate fall protection system, exposing employees to fall hazards.  High Innovation Corporation was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1162921, citation number 1, item number 1 and was affirmed as a final order on 9/22/2016, with respect to a workplace located at 449 Hamilton St, Norristown, PA.
