29 CFR 1910.27(b)(1)(ii): (ii) The employer must ensure that no employee uses any anchorage before the employer has obtained written information from the building owner that each anchorage meets the requirements of paragraph (b)(1)(i) of this section.  The employer must keep the information for the duration of the job.    At this Ten Story Building: On or about March 21, 2018, and at times prior to employees were exposed to fall hazards while performing window washing activities without being informed that each anchor point meets the requirements of 5,000 pound capacity in any direction for each employee attached prior to doing the work.

29 CFR 1910.27(b)(2)(iv): (iv) The rope descent system shall  be inspected at the start of each workshift that it is to be used.  The employer failed to ensure damaged or defective equipment was removed from service immediately and replaced.      a) Window Area, On or about March 21, 2018, and at times prior to thereto, worker(s) who use body harnesses and landyards were exposed to fall hazards when the personal protective equipment showed evidence of excessive wear, rust, illegible labeling and fraying and were not taken out of service.      b) Window Area, On or about March 21, 2018, and at times prior thereto, worker(s) who use carabiners were exposed to a fall hazard. The carabiners showed evidence of excessive wear, no load bearing markings and chips on carabiner while utilizing as part of their safety system.       c) Window Area, On or about March 21, 2018, and at times prior thereto, worker(s) who use and operate a descent chair as a fall protection system were exposed to fall hazards when the descent chair did not meet the requirements of the manufacture and was not taken out of service.

29 CFR 1910.27(b)(2)(vi): Each employee uses a separate, independent personal fall arrest system which did not meets the requirements of subpart I;       Window Washing Area, On or about March 21, 2018, and at times prior to thereto, worker who uses a rope descent system was exposed to fall hazards when a worker was seen descending down from the 6th floor without wearing an independent personal protective system.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.      At this Ten Story Building: On or about March 21, 2018, and at times prior to employees were exposed to fall hazards while performing window washing activities without being trained in how to inspecting, operating, maintaining, the correct and disassembling the fall protection systems.
